Monitor was informed of the histopathology allegations by John Savage during a routine telephone call. John Savage had known about them for at least a year as he was copied on the 15th June 2008 letter to Graham Rich. Yet neither thought it necessary to inform Monitor as a Foundation Trust regulator. UBHT became a Foundation Trust on 1st June 2008 and was thereafter known as UHB.
